要提高对弱势群体的社会支持的效率 ,必须关注支持的发送与传递的实践过程。以社会网络理论为基础的社会支持网的工作模式 ,把支持与被支持视为一个持续的互动过程 ,以整合社会支持资源、建立弱势群体自身的社会支持网为目标 ,协助解决支持传递的实践过程中出现的各种问题 ,并及时跟进各种措施 ,以推动弱势群体的主动参与和自我发展。为此 ,以社区为基地 ,培育专业社会工作者与公益团体 ,树立资源共享的理念等是需要解决的实践问题。

Emphasis should be placed on the process of sending and transferring social support so as to enhance the efficiency of social support. This paper proposes a pattern of social support network based on the theory of social network, which regards supporting and being supported as a continuing interactive process, and aims at integrating all the resources of social support and organizing a social support network among the disadvantaged themselves. Such a network can help solve all kinds of problems encountered in the process of transferring social support and follow up various measures so that active participation and self-actualization of the disadvantaged may be enhanced. Therefore, it is necessary to solve such problems as setting up bases in communities, training professional social workers, establishing public welfare organizations, and promoting the idea of sharing resources, etc.
